To make an appointment, fill out the form below and a scheduler will call you. We will make every effort to contact you the next business day, but in busy periods it may take up to three days. Please have your medical insurance and vision plan information available when you are called for your appointment (plan name, ID number and subscriber information).
In case of emergency, please do not use email. Call our office immediately.
If you have cold or flu symptoms, please DO NOT come to the office for your appointment. You may call to cancel at any time with NO cancellation charge. You may be asked to wear a mask if you show symptoms at your appointment.
